我先了A .说是主句的主语也是状主的主语.我想form需要填好.那主语是form,前面的状语是否要用被动形式?
而C.选项为何要用ing的一般式呢?

因为它说出了"申请",所以就跟填表格练习较为紧密.
2.once 连词.后跟一个句子.这里省略了主语和谓语:

a和c句子本身就是错的.b也是错的,因为2个小句子中间用句号,不能用逗号.
